Job summary

The MDS Coordinator at Advanced Center for Nursing and Rehabilitation in New Haven, CT, will oversee the Resident Assessment Instrument (RAI) process, perform MDS assessments, and drive care planning to ensure compliant, quality resident care.

You will coordinate with nursing, therapy, and clinical staff, ensure timely submissions to CMS and state agencies, and optimize documentation to support appropriate reimbursement under PDPM and MDS 3.0.

Qualifications

• Active RN license required; LPN/LVN may be considered.
• Certification such as RAC-CT preferred.
• 1–3 years in long-term care, skilled nursing, or rehabilitation.
• Strong knowledge of MDS 3.0, RAI, CMS regulations, and PDPM.
• Proficient in electronic health records (EHR) systems.
• Excellent organizational, analytical, and communication skills.

Responsibilities

• Coordinate and complete MDS assessments within regulatory timelines.
• Develop individualized care plans based on assessments.
• Ensure compliance with CMS and state regulations; participate in surveys.
• Support reimbursement processes through accurate coding (RUG/PDPM).
• Participate in quality improvement initiatives and infection control as needed.
