Job summary

LaGrange Health and Rehab seeks an experienced RN/LPn to oversee the resident assessment (MDS) process and care planning. You will coordinate multidisciplinary input, ensure regulatory compliance, and support family involvement in planning.

Role requires leadership, travel for site coverage, and ability to manage multiple projects while maintaining high accuracy and timely submissions.

Qualifications

• Must be a Registered Nurse or Licensed Professional Nurse in good standing.
• Minimal 2 years of MDS/care plan and Long Term Care experience.
• Ability to relate positively to residents, families, and regulators and to work cooperatively with others.
• Experience in performance management and effective leadership.
• Ability to complete multiple projects.
• Must be able to travel, including overnight stays.
• Must meet health regulations and pass post-employment screening and background checks.
• Capable of performing essential job functions with or without accommodation.

Responsibilities

• Oversee and coordinate development and completion of resident assessments (MDS) per regulations.
• Assist in developing comprehensive resident assessments and care plans.
• Disseminate new materials involving RAI processes.
• Facilitate involvement of health professionals to improve resident function.
• Enable family participation in care planning.
• Ensure face validity of all MDS before submission and oversee timely electronic submissions.
• Review validation reports and take appropriate actions.
• Serve on committees as assigned by DON or Regional Director.
• Conduct monthly random sample reviews of MDS for accuracy and consistency.
• Collaborate with product champions to train new associates on clinical software.
