Qualifications

• Must hold a current nursing license in the state of employment.
• Experience with MDS 3.0 and CAAs is required.
• Ability to develop individualized resident care plans.

Responsibilities

• Participates in Resident Assessment Process via MDS items and CAAs.
• Contributes to individualized care plans and projected outcomes.
• Ensures compliance with Federal and State regulations.
