Job summary

Journey in St. Matthews, KY seeks a licensed nurse to join the Care Team as an MDS Assistant. You will participate in the Resident Assessment Process by completing MDS 3.0 items and CAAs, and help develop individualized care plans that address resident needs and outcomes.

You will work under federal and state regulations, ensuring accurate data collection and contributing to quality outcomes. A current nursing license is required, with knowledge of MDS 3.0 highly preferred.

Qualifications

• Licensed professional nurse with current license in the state of employment.
• Knowledge of MDS 3.0 and CAAs.

Responsibilities

• Complete relevant items on the MDS and CAAs as part of Resident Assessment Process.
• Assist in developing individualized resident Care Plans.
• Identify residents' needs and projected outcomes per federal/state regulations.

The MDS Assistant participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.

Required Qualifications

• A professionally trained Licensed Nurse that has earned a certificate/diploma or degree in nursing.
• Must hold a current nursing license in the state of employment.
• Working knowledge of the MDS 3.0 (current version of minimum data set).
