Job summary

Journey in Saint Matthews, KY is seeking a professional RN to join our Care Team as an MDS Coordinator. You will conduct and coordinate MDS assessments, develop individualized care plans, and ensure timely updates for changes in residents' conditions, in alignment with federal and state guidelines.

You will collaborate across departments to support resident care outcomes, ensure accurate documentation of assessments, and participate in care planning meetings.

Qualifications

• Must hold a current RN license in the state of employment.
• Working knowledge of the MDS 3.0 (current version of minimum data set).

Responsibilities

• Conduct and coordinate MDS assessments, develop individualized care plans, and ensure timely updates for any significant changes in residents' conditions.
• Ensure accurate completion and submission of MDS assessments in accordance with Medicare/Medicaid guidelines and state/federal regulations.
• Facilitate care plan meetings, maintain effective communication across departments, and support continuous improvements in resident care outcomes.
