Journey in Saint Matthews, KY is seeking an MDS Assistant to join our local Care Team. The role supports the Resident Assessment Process, helps develop personalized Care Plans, and ensures regulatory compliance.

As a professionally trained RN with MDS 3.0 knowledge, you will collaborate with residents and families to deliver high-quality outcomes and uphold Journey's commitment to compassionate, community-centered care.

Qualifications

• Registered Nurse with a nursing certificate, diploma, or degree.
• Current RN license in the state of employment.
• Working knowledge of the MDS 3.0 (current version).

Responsibilities

• Participate in the Resident Assessment Process via MDS 3.0 and CAAs.
• Develop individualized resident Care Plans with identified needs and outcomes.
• Ensure compliance with Federal and State regulations in daily practice.

MDS Assistant participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.

Required Qualifications

• A professionally trained Registered Nurse that has earned a certificate/diploma or degree in nursing.
• Must hold a current nursing license in the state of employment.
• Working knowledge of the MDS 3.0 (current version of minimum data set).
