Journey is seeking a dedicated MDS Assistant to support resident assessments through MDS 3.0 items and related CAAs. You will help craft individualized Care Plans and ensure regulatory alignment across federal and state requirements.
As a licensed nurse, you will collaborate with the Care Team to deliver high-quality outcomes for residents and families in a community-centric setting.

MDS Assistant participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.
