Job summary

Journey, your local elder care partner in St. Matthews, KY, is seeking an MDS Assistant to support the Resident Assessment Process and develop individualized Care Plans.

In this role you will complete the MDS 3.0 items, identify resident needs and outcomes, and work with the Care Team to deliver quality, compliant care. Starting with a registered nurse background and current KY license, you’ll join a compassionate team.

Qualifications

• Licensed Registered Nurse with current state license.
• Working knowledge of the MDS 3.0 process.
• Ability to participate in care planning and resident assessments.

Responsibilities

• Participate in the Resident Assessment Process via MDS items and CAAs.
• Develop individualized resident Care Plans and identify needs and outcomes.
• Collaborate with the Care Team to meet federal/state regulations.

The MDS Assistant participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.

Required Qualifications

• A professionally trained Registered Nurse that has earned a certificate/diploma or degree in nursing.
• Must hold a current nursing license in the state of employment.
• Working knowledge of the MDS 3.0 (current version of minimum data set).
