Job summary

Access Community Health Network is seeking a Consortium Peer Advocate, MCH to co-chair the Westside Healthy Start Community Consortium and manage a small caseload of patients. The role emphasizes helping patients access maternal health services, providing health education and coaching, and supporting program planning and evaluation.

• High school diploma required.
• Past or current Westside Healthy Start program participant.
• CPR/BLS certification preferred.

Responsibilities

• Assist in developing, executing, and documenting an annual action plan addressing barriers to care and social determinants of health.
• Attend and participate in Westside Healthy Start Consortium meetings and events.
• Provide case management to patients enrolled in Westside Healthy Start.
• Conduct follow-ups to ensure patients access services and complete care plans.
• Support care team operations and contribute to organizational metrics.
