Job summary

CHAS Health is seeking a dedicated professional to improve community health by expanding maternity and pediatric access to care. You will perform intakes, document progress, and advocate for patients within the First Steps program, collaborating with CHAS Health teams to ensure effective outcomes.

Qualifications

• Minimum of two years in health or social services.
• Bachelor’s degree in social work, behavioral sciences, psychology, child development, or mental health required.
• Excellent oral and written communication.
• Compassion for patients.
• Computer skills required.
• Ability to work under tight time constraints and changing priorities.
• Self-motivated, able to work independently or in a team.
• Valid driver’s license and insurance.
• BLS (CPR/AED) required.
• Commitment to a safe and respectful environment.

Responsibilities

• Conduct intakes and assessments per First Steps guidelines.
• Document progress and follow up per billing guidelines.
• Advocate and refer patients to community resources.
• Counsel patients and families on healthcare recommendations.
• Assist with non-clinical problems under supervision.
• Collaborate with CHAS Health teams and outside entities.
• Teach maternal and infant care as required.
• Assist in special studies and resource development.
• Participate in community outreach activities.
• Provide at-home visits when needed.
• Perform other duties supporting CHAS Health Mission.
