CHAS Health is seeking a dedicated team member to improve maternal and infant health access within the communities we serve. You will conduct intakes, assess needs, and connect patients with appropriate resources under the First Steps guidelines.

Responsibilities include coaching families, coordinating with multidisciplinary teams, performing home visits when needed, and supporting outreach activities to promote healthy birth outcomes. A valid driver's license is required.

• Excellent oral and written communication skills.
• Compassion for patients.
• Computer skills required.
• Valid driver’s license and insurance required.

Responsabilités

• Conducts intakes and assessments of client needs under First Steps guidelines.
• Advocates and refers patients to appropriate community resources.
• Counsels and aids patients and families to understand healthcare options.
• Assist in non-clinical problems affecting patient care under supervision.
• Works with CHAS Health teams and outside entities to improve birth outcomes.
• Performs at-home visits for patients as needed.
