Job summary

Qualifications

• BSN from an accredited college and GA RN license.
• Current American Heart Association Healthcare Provider BLS/BCLS certification.
• Minimum of 2 years of related nursing experience.

Responsibilities

• Coordinate care for pregnant and postpartum participants in Vida Maternal Care Support.
• Monitor health data and remote monitoring results per protocol.
• Identify barriers to care and coordinate referrals and follow-up.
• Schedule and track appointments, referrals, and inter-provider communications.
• Maintain regular contact with participants via phone, text, and program platform.
• Document contacts, monitoring results, referrals, and outcomes accurately.
