Job summary

Augusta University is seeking a Nurse Navigator to promote high-quality, patient-centered maternal health care across Vida programs. The role coordinates assessments, plans care, and supports access to preventive services, remote monitoring, and social supports.

Travel within the service region may be required to engage communities and partners. The Nurse Navigator collaborates with multidisciplinary teams, manages documentation, and ensures adherence to program protocols while supporting

Qualifications

• Bachelor of Science in Nursing from an accredited college or university.
• Georgia RN license in good standing.
• Current American Heart Association Healthcare Provider BLS/BCLS certification.
• Minimum of 2 years of related nursing experience.

Responsibilities

• Facilitate comprehensive care coordination for pregnant and postpartum participants.
• Monitor health information and escalate abnormal results to clinical providers.
• Identify barriers to care and coordinate appropriate referrals and follow-up.
• Coordinate appointments, referrals, and communications with care providers.
• Maintain regular participant contact via phone, text, and platform.
• Document contacts, monitoring results, referrals, and outcomes per program procedures.
