Job summary

Erie Family Health Centers in Chicago seeks a Birthing Families Navigator to join the Case Management Department. This role helps identify clients, delivers supportive services, and coordinates care from prenatal through early childhood with providers and care teams.

You will perform outreach, assessments, referrals, home visits as needed, and maintain documentation in funder data systems and our electronic health record. Bilingual English/Spanish is required.

Qualifications

• Associate’s degree in social work, child development, community health, or related field required.
• Bachelor’s degree in social work, child development, or related field preferred.
• A minimum of two years' work experience in health, social or community services preferred.

Responsibilities

• Provides coordination of primary care (prenatal, post-partum, and/or pediatric care) and social services, outreach, follow ups, risk assessments, referrals, home visitation as needed, and tracking of clients’ appointment failures.
• Collaborates with fellow Birthing Families Navigators to ensure coverage and presence throughout the day.
• Collaborates with client’s provider and care team.
• Maintains familiarity with each client’s care plan and facilitates referrals and follow-up.
