Job summary

Care Medical Practice seeks a Maternal Health Navigator to support the Healthy Start Buffalo program through administrative coordination, enrollment navigation, and data-quality duties. You will help connect families to services and ensure program requirements are met.

You will collaborate with Healthy Start staff, maternity clinical teams, and community partners to ensure accurate documentation, timely referrals, and smooth operations in a fast-paced, community-based setting.

Qualifications

• Bachelor’s degree in Public Health, Human Services, Health Administration, Social Work, or related field or equivalent experience.
• 3–5 years in healthcare, maternal health, or community program setting.
• Experience with intake, enrollment, scheduling, referrals, or program coordination.
• Strong interpersonal and communication skills, respectful engagement with participants and families.
• Demonstrated organizational skills in a fast-paced environment.
• Proficiency with Microsoft Office; MEDENT or similar EHR is preferred.
• Familiarity with maternal/infant health services and community resources.
• Ability to maintain accurate documentation and support HRSA grant reporting.

Responsibilities

• Support daily operations of Healthy Start Buffalo program, including administrative coordination.
• Handle participant intake, enrollment initiation, and follow-up for referrals.
• Assist with resource navigation, focusing on scheduling transportation for appointments.
• Manage inbound/outbound participant communications for scheduling and engagement.
• Coordinate with care teams to support triage and service connections.
• Maintain participant records across required systems (e.g., MEDENT).
• Support data quality and HRSA reporting requirements.
• Prepare and distribute program materials; participate in outreach events.
• Attend meetings and provide admin or documentation support.
• Conduct outreach to group education participants.
