Job summary

The Maternal and Family Health Services seeks a full-time Maternal Health Advocate to connect clients with MFHS and community programs in Bradford and Tioga counties, supporting prenatal, perinatal, and postpartum care through education and navigation.

You will build relationships with clients and partners, facilitate referrals, assist with appointments and transportation, and help enroll in public programs, aiming to promote integrated care and healthier birth outcomes.

Qualifications

• High School Diploma or GED required; Bachelor’s degree preferred.
• CHW certification preferred; must be obtained within one year if not held.
• One year experience in community settings with diverse families.
• Knowledge of psychosocial, economic, and cultural determinants to health.
• Excellent communication and interpersonal skills.
• Bilingual English/Spanish preferred.
• PA driver’s license or reliable transportation.

Responsibilities

• Care navigation to connect clients to care and health/social services.
• Assist with insurance applications, appointments, and referrals.
• Prepare clients for appointments with required documentation.
• Promote service integration across MFHS programs.
• Engage with community partners and build partnerships.
• Assist with outreach at events and conferences.
• Administrative support including translation and data reporting.
• Support client education resources.
• Promote organizational goals and assist with car seat installation.
• Distribute supplies and document data for reporting.
