Responsibilities

• Provide 1:1 peer support to individuals and families in various community settings.
• Assist navigating systems of care for needs related to mental health and substance use.
• Identify strengths, barriers, and goals with participants and families.
• Collaborate with care teams to develop recovery plans and crisis plans.
• Provide coaching, education, and practical support to strengthen wellness and self-advocacy.
• Document interactions and progress in required records within 24 hours.
