Qualifications

• Current, unrestricted license as a Registered Nurse in Minnesota.
• Associate degree in nursing.
• Current Basic Life Support certification.
• At least two years of professional nursing experience.
• Demonstrated ability to conduct nursing assessments, provide patient education, coordinate care, and document services with detailed nursing notes.
• Excellent verbal and written communication.

Responsibilities

• Coordinate prenatal, perinatal, postpartum, newborn and pediatric care within the Maternal Child Health Program.
• Maintain patient tracking for engagement, gestational age, postpartum follow-up, well-child visits and program measures.
• Complete accurate documentation of patient encounters and close OB charts per protocol.
• Provide nurse-led maternal and child health visits using standing orders and protocols.
• Collaborate with cross-disciplinary teams to coordinate care and ensure timely access to services.
