Qualifications

• Active and unrestricted RN license in Minnesota.
• Public health nurse registration preferred.
• Clinical experience in community health and outreach nursing.
• Experience with chemically dependent adults and harm reduction principles.
• Excellent verbal and written communication skills.
• Strong organizational, problem-solving, and analytical skills; able to manage priorities and workflow.
• Ability to work independently and as a member of teams.
• Acute attention to detail.
• Good judgement with timely and sound decisions.
• Ability to plan and organize projects; versatile and adaptable to changing priorities.

Responsibilities

• Provide professional clinical care and care coordination in community and clinic settings.
• Deliver wound care, foot care, vaccinations, phlebotomy, education, vital signs, and standing orders.
• Coordinate and assist in provider visits via telehealth or in-person; trauma-informed care.
• Utilize harm reduction strategies; provide naloxone trainings and safer use supplies.
• Assess unmet medical and social needs; connect to primary care and resources; reduce barriers.
• Work with clinic staff to identify high-risk or out-of-care individuals; liaison to care.
• Document encounters accurately in the EMR per clinic protocol.
• Assist in care transitions and connect clients to external services.
• Lead outreach and partnerships with community organizations; engage in education and referrals.
• Document data, plans, and patient progress in the EMR.
