Job summary

You will coordinate patient scheduling, maintain records, and manage admissions while counseling patients and families about treatment and conditions.

You will act as a liaison among case managers, home health nurses and utilization review, coordinating patient services and care to support optimal outcomes.

Qualifications

• Valid RN license required.
• Experience with patient assessment and care planning.
• Strong communication and teamwork abilities.
• Proficiency with computer systems and EHR software.
• Ability to document care accurately in EMR.

Responsibilities

• Provides patient care by performing assessments, planning, implementing and evaluating treatments and documenting care in the EMR.
• Coordinates delivery of care within the practice and across settings; refers patients to needed services.
• Fosters interdisciplinary collaboration to improve quality, safety and outcomes.
• Communicates openly with patients, families and health care professionals about care and roles.
• Ensures access to safe, effective, timely and equitable care with cost-effective outcomes.
• Mentors new staff and participates in orientation and education programs.
• Maintains patient confidentiality and reports concerns as appropriate.
• Demonstrates exceptional customer service standards.
• Engages in initiatives to maintain a safe and compliant clinical environment.
• Follows policies for infection control and safe handling of waste.
