Job summary

Access Community Health Network in Chicago seeks a Consortium Peer Advocate, MCH, to support Westside Healthy Start patients and co-chair the consortium efforts. This grant-funded role focuses on empowering families, coordinating care, and connecting clients to maternal health services.

You will work closely with the Program Coordinator, MCH, provide education, follow-up, and help advance community partnerships while reporting on program activities and outcomes.

Qualifications

• High school diploma required.
• Past or current Westside Healthy Start participant.
• CPR/BLS certification required.

Responsibilities

• Assist in developing and documenting an annual action plan to address barriers to care.
• Attend Westside Healthy Start meetings and events.
• Provide case management to assigned patients under the Westside Healthy Start curriculum.
• Coordinate referrals and connect patients to community resources.
