Aufgaben

• Provide 1:1 peer support to individuals and families in homes and community-based settings, using lived experience to build trust and engagement.
• Help navigate substance use, mental health, and other systems of care to ensure needs are addressed.
• Identify strengths, barriers, needs, goals, and priorities with participants and families.
• Collaborate with care teams to develop plans of care, crisis plans, and recovery supports.
• Provide coaching, education, and practical support to strengthen wellness and self-advocacy skills.
• Link participants to services and community resources to build support networks.
• Attend appointments and community activities with participants to promote engagement.
• Complete documentation and progress notes timely and accurately in EMR per policy.
• Participate in supervision, staff meetings, trainings, and required meetings.
• Assist with coverage for co-workers as needed.
