Job summary

Qualifications

• Bachelor’s Degree required from an accredited university with emphasis in psychology, social work, health education, interdisciplinary sociology, early childhood, child development or special education.
• One year of relevant experience required with young children and families in a teaching, case management or counseling role, or with individuals with special needs and/or developmental delays of any age in a case management.
• Knowledge of various data systems and the ability to learn, navigate, and work effectively within database systems.
• Skill in completing assignments accurately and with attention to detail.
• Ability to analyze, organize and prioritize work under pressure while meeting deadlines.
• Ability to process and handle confidential information with discretion.
• Ability to work evenings, nights, and weekends as necessary.

Responsibilities

• Conducts patient evaluations to obtain information on medical and mental health to develop an effective treatment plan.
• Ensures patients are enrolled in insurance and social service programs for which they are eligible.
• Learns enrollment processes to transition care between programs and insurance plans.
• Identifies and resolves barriers to care to improve health outcomes.
• Assists with patient-provider communication and medication adherence education.
