Job summary

The Florida Department of Health seeks a Human Services Program Consultant II to provide specialized case management for pregnant women, inter-conception women, and families in the Bellies, Babies and Beyond program. The role requires outreach, assessments, counseling, and coordination of services to improve health outcomes.

Responsibilities include care coordination, referrals, and building on family strengths.

Qualifications

• A bachelor’s degree from an accredited college or university in social work, social sciences, or a health-related field.
• Minimum of three years of verifiable professional experience in providing social work, health education, nursing or counseling services.
• Fluent in speaking Creole and communicating in proper English.
• At least one year of verifiable professional experience conducting home visits with clients.
• One year of experience conducting interviews or assessments in Health or Social Services.

Responsibilities

• Provides case management services to pregnant women and inter-conception women to coordinate service delivery.
• Establishes rapport and builds relationships with families from program entry.
• Identifies needs and plans with families to promote health, literacy and self-sufficiency.
• Coordinates and documents services in BBB service plan and CareWare system.
• Collaborates with providers to ensure continuity of care and advocates for participants.
