Responsibilities include responding to referrals, maintaining records, and sustaining a caseload of 40+ consumers monthly.

Qualifications

• 2 years of public health/community development experience.
• Bilingual preferred.
• Able to manage a caseload of 40+ monthly.
• Excellent written and oral communication.
• Knowledge of HIPAA and confidentiality.
• Proficient in MS Office.

Responsibilities

• Respond to Healthy Start referrals promptly.
• Educate on childbirth, breastfeeding, women's health, and parenting.
• Explain Healthy Start services and identify risk factors.
• Develop relationships with primary health care providers.
• Provide care coordination and referrals.
• Maintain progress notes and documentation in consumer files.
• Manage a caseload of 40+ clients per month.
• Attend meetings, trainings and in-service as assigned.
• Ensure client records are accurate and updated.
• Link clients to services (WIC, food banks, housing, mental health, etc.).
