Job summary

Qualifications

• Bachelor's degree required in a related field.
• Minimum 2 years of public health/community development experience.
• Bilingual preferred; ability to work with diverse populations.
• CPR/First Aid and TB test certifications required.
• Proficient with MS Office and data entry in health systems.

Responsibilities

• Coordinate services for women and infants referred to Healthy Start.
• Provide education on childbirth, breastfeeding, and parenting.
• Develop relationships with primary health care providers.
• Maintain updated progress notes and consumer files per guidelines.
• Manage a caseload of 40+ consumers per month.
