Job summary

Qualifications

• Bachelor's degree in a related field required.
• 2 years public health/community development experience.
• Bilingual is preferred.

Responsibilities

• Coordinate services for women and infants referred to Healthy Start.
• Maintain updated case records and progress notes.
• Provide education on childbirth, breastfeeding, and parenting.
• Develop relationships with primary health care providers.
• Maintain a caseload of ~40+ consumers per month.
• Prepare administrative reports in a timely manner.
• Ensure HIPAA compliance and confidentiality.
