Job summary

Sun River Health is seeking a full-time MAT Licensed Practical Nurse to join our Coram, Long Island team in New York. The MAT LPN will work within a multidisciplinary team to plan and evaluate patient care, uphold infection control, and assist in crisis management while advocating for patients.

Requires NYS LPN license and CPR with 1 year of experience; a 2–4 year preference; driving license may be needed for community referrals. Sign-on bonus offered for eligible external applicants.

Qualifications

• Minimum education: High School Diploma or GED.
• Preferred education: Vocational/ Technical School or Diploma Program - Nursing.
• Minimum work experience: 1 year.
• Preferred work experience: 2-4 years.

Responsibilities

• Maintain patient medical records and vitals; document medications and treatments.
• Perform in-house labs and collect specimens; ensure quality control.
• Prepare equipment and assist physicians during treatments and examinations.
• Complete and update logs; manage shipments and vaccines.
