Job summary

Qualifications

Bachelor’s degree in a social or behavioral science field.
At least one year of experience providing case management services.
Ability to complete required training within six months of hire.

Responsibilities

Conduct client outreach, assessments, and individualized service planning.
Provide education on recovery, treatment options, and support services.
Coordinate care and make referrals to appropriate agencies and providers.
Serve as the primary contact for referrals from local health departments.
Collaborate with community partners, including courts, DCBS, law enforcement, and shelters.
Maintain accurate documentation and case records.
Support clients in navigating complex systems and accessing services.
