Qualifications

Responsibilities

• Collaborate with the clinical team to engage, educate, communicate, and coordinate care for clients in MAT programs.
• Conduct outreach and follow-up for services including crisis incidents, missed appointments, and hospital discharges.
• Coordinate care with family, providers, and state agencies and share relevant clinical information as needed.
• Participate in clinical staffing, case conferences, and family-inclusive meetings.
• Provide excellent customer service to clients, agencies, stakeholders and staff.
