Qualifikationen

• Associates degree preferred, 3 years working in a related field required.
• Knowledge of community services, primary health care, eligibility criteria and intake processes.
• Understanding of mental health, mental disabilities, and substance use treatment programs.
• Ability to interview and assess clients with appropriate assessment tools.
• Must have a valid driver’s license and reliable transportation.
• Background check required after hire.

Aufgaben

• Facilitate and complete regular treatment planning, screening tools, and reassessment.
• Accept newly assigned cases after intake is completed.
• Coordinate services while developing therapeutic rapport with clients.
• Document modifications to the treatment plan as necessary.
• Document all service contacts on a timely basis.
• Meet regularly with supervisor to review case management and administrative tasks.
• Provide staff training in areas of expertise as requested by supervisor.
• Participate in community education activities as requested by supervisor.
• Position is community-based with some meetings in the office.
