Job summary

The Home for Little Wanderers is hiring a Mental Health Intensive Care Coordinator in the Dorchester/Boston area. This full-time role develops and implements Individual Care Plans for MassHealth youth with serious emotional disturbance, coordinating with families, schools, and providers to ensure comprehensive care.

The position requires a Master’s degree in a mental health field and Massachusetts CANS certification, with preferred bilingual abilities.

Qualifications

• Master’s degree in a mental health field from an accredited college or university.
• Experience navigating child/family-serving systems and advocating for families.
• Must maintain Massachusetts CANS certification.
• Second language skills (Spanish, Vietnamese, or Haitian Creole) preferred.
• Computer literacy and ability to learn new software applications.

Responsibilities

• Act as liaison with collateral agencies and programs to support children and families.
• Identify and assist youth and families to obtain and monitor services.
• Perform initial assessments and develop Individual Care Plans.
• Maintain relationships with local providers to facilitate referrals.
• Communicate with clients and providers; attend case conferences and supervision.
