Job summary

The Home for Little Wanderers in Boston, MA is seeking a caregiver/parent with experience to support families and navigate child-serving systems. This role involves guiding and coaching parents, coordinating with the Care Planning Team, and delivering home-based support within a dynamic clinical team.

Responsibilities include weekly in-person visits, collaboration with other health providers, and documenting progress in MassHealth-compliant notes while maintaining a flexible, family-centered

Qualifications

• Must have experience as a caregiver/parent of a child or youth with special needs.
• Experience with significant mental health needs preferred.
• Must be able to navigate child-serving systems with guidance.

Responsibilities

• Work one-on-one with parents/caregivers to provide guidance, coaching, education, and support through care planning.
• Participate in the Care Planning Team and clarify Family Support goals.
• Collaborate with families and other providers to implement Individual Care Plans.
• Conduct weekly in-home or community visits and provide ongoing telephone support.
• Document progress notes and maintain caseload as required.
