Job summary

The Home for Little Wanderers in the Boston area is seeking a Full-Time Mental Health Intensive Care Coordinator. You will develop and implement Individual Care Plans and risk management/safety plans for MassHealth youth with serious emotional disturbance, under supervision of senior staff.

You will liaise with collateral agencies, monitor services, document plans, and attend team meetings to ensure coordinated care and safety for families.

Qualifications

• Master’s degree in a mental health field from an accredited college or university.
• Experience navigating child/family-serving systems and advocating for families involved with behavioral health systems.
• Must maintain certification in the Massachusetts CANS.
• Knowledge of local communities; bilingual preferred but not required.
• Computer literacy including Microsoft Office and Internet Explorer.

Responsibilities

• Act as a liaison with collateral agencies and programs; educate families on advocacy.
• Identify and assist youth to obtain and monitor services (medical, educational, social, therapeutic).
• Develop and monitor Individual Care Plans and Transition Plans.
• Maintain relationships with local providers to facilitate referrals and coordinated care.
• Communicate with clients and providers; schedule and attend case conferences.
