Job summary

The Home for Little Wanderers in Roslindale, Boston, seeks a Full-Time Intensive Care Coordinator to develop and implement Individual Care Plans and risk management for MassHealth youth with serious emotional disturbance.

You will work with collateral agencies to ensure timely access to medical, educational, social, and therapeutic services; assess new clients; and document plans and progress while coordinating with providers and families.

Qualifications

• Bachelor’s degree in a mental health field from an accredited college or university.
• Experience navigating child/family-serving systems and advocating for families involved with behavioral health.
• Must maintain certification in the Massachusetts CANS.
• Knowledge of a second language such as Spanish, Vietnamese, or Haitian Creole desired.

Responsibilities

• Act as a liaison with collateral agencies and programs; educate families and advocate for services.
• Identify and assist youth and families to obtain and monitor medical, educational, social, therapeutic, and community supports.
• Perform initial assessments; develop and monitor Individual Care Plans and Transition Plans.
• Maintain relationships with local providers to facilitate referrals and coordinated care.
• Communicate with client, collaterals, and providers; coordinate meetings and ensure timely documentation.
• Document assessments and treatment plans meeting MassHealth requirements; participate in supervision and training.
