We are looking for an energetic, personable Community Liaison to build relationships throughout the community and help connect families in need of care with our facility.
The Community Liaison is responsible for growing census and building the facility's reputation by developing and maintaining strong relationships with hospitals, case managers, physicians, families, and community organizations. This is a relationship-driven, outward-facing role that spends the majority of its time in the field rather than at a desk.
Visit local hospitals regularly to build and maintain relationships with discharge planners, case managers, and social workers.
Serve as the primary point of contact for hospital case managers coordinating patient discharges and referrals to the facility.
Meet with families to answer questions, address concerns, and guide them through the admissions process with warmth and professionalism.
Represent the facility at community events, health fairs, senior expos, and other outreach opportunities to raise awareness and generate referrals.
Develop and maintain relationships with physicians, assisted living communities, senior centers, churches, and other referral sources.
Track referral activity and follow up promptly on leads and inquiries.
Coordinate facility tours for prospective residents and their families.
Distribute marketing materials and represent the facility's brand and values consistently in the community.
Collaborate with the admissions and administrative team to ensure a smooth transition for new residents.
Report on outreach activity, referral trends, and market feedback to leadership.
Stay informed on local competitors and market conditions to identify new opportunities for growth.
High school diploma required; Bachelor's degree in marketing, healthcare administration, or related field preferred.
Prior experience in healthcare marketing, sales, or community outreach strongly preferred (skilled nursing or senior care experience a plus).
Excellent interpersonal and communication skills, with the ability to build trust quickly with families, clinicians, and case managers.
Comfortable with frequent travel within the community (hospitals, events, referral sources).
Self-motivated, organized, and able to manage a flexible schedule, including occasional evenings or weekends for events.
Valid driver's license and reliable transportation required.
Compassionate, professional demeanor when working with families during a difficult and emotional time.
