HCA Houston Healthcare Clear Lake is seeking a Manager Case Management to lead the case management team and coordinate high-quality patient care across the hospital. The role emphasizes staff supervision, care coordination, and optimization of patient flow within a Level II trauma center and heart hospital environment.

The position requires an active Texas RN license (or multi-state compact) and a BSN, with a minimum of five years of RN case management experience.

Formación

• Registered nurse license in Texas or multi-state compact required
• BSN degree required
• 5 years RN case management experience preferred
• InterQual experience preferred
• Case Manager Certification preferred

Responsabilidades

• Oversee and coordinate case management services for patients
• Collaborate with clinical teams to develop and implement care plans
• Ensure timely patient discharge planning and resource utilization

Job Summary and Qualifications

What qualifications you will need:

Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date)

Graduate of an accredited school of professional nursing is required

BSN required

5 years RN Case Manager experience

Case Manager Cert preferred

InterQual experience preferred
