HCA Houston Healthcare North Cypress in Cypress, TX seeks a Nurse Manager RN - Case Management to lead case management staff and oversee patient care coordination across service lines, including Chest Pain Center, Bariatric Center, and Primary Stroke Center. You will supervise staff, optimize utilization, and ensure timely discharge planning to meet each patient’s medical needs.

Applicants should have an accredited nursing degree (BSN preferred), Texas RN license, and case management experience.

Qualifications

• Graduate of an accredited diploma, associates, or baccalaureate degree registered nursing program.
• BSN degree is preferred.
• Current Texas RN license or Compact license with requirement to obtain Texas license within 90 days.

Responsibilities

• Lead and supervise the Case Management staff while acting as a key communicator, planner, assessor, implementer, and evaluator.
• Review patient hospitalizations to verify medical necessity and appropriate length of stay.
• Optimize service utilization by ensuring timely and efficient clinical services during the patient’s stay.
• Coordinate discharge planning by identifying and managing ongoing post‑hospital care needs.
• Provide age‑specific oversight across neonates through geriatric patients.
• Maintain cross‑functional knowledge of physical, psychological, educational and safety criteria for diverse demographics.

Work schedule: Monday-Friday – additional coverage as needed.

Based at HCA Houston Healthcare North Cypress in Cypress, Texas, this leadership role oversees care coordination within a 163-bed general acute care hospital renowned for clinical excellence. The Nurse Manager will collaborate with specialized medical teams across the facility's prominent service lines, including its accredited Chest Pain Center, Bariatric Center of Excellence, and certified Primary Stroke Center. To support comprehensive utilization review and seamless transitions of care, the facility features robust clinical departments spanning heart and vascular surgery, neurological care, advanced orthopedic care, surgical oncology, and a 23-bed inpatient rehabilitation unit. Managing case assignments in tandem with these key service areas ensures that our patients receive high-quality, continuous, and timely care matching their precise medical needs.

Job Summary And Qualifications

What you will do in this role:

• Lead and supervise the Case Management staff while acting as a key communicator, planner, assessor, implementer, and evaluator.

• Optimize service utilization by ensuring all clinical services are rendered timely and efficiently throughout the patient's stay.
• Coordinate discharge planning by proactively identifying and managing ongoing patient care needs for life after hospitalization.
• Deliver age‑specific oversight across all populations, from neonates and infants to adult and geriatric patients.
• Maintain cross‑functional knowledge regarding physical, psychological, educational and safety criteria for diverse patient demographics.

What qualifications you will need:

• Graduate of an accredited diploma, associates, or baccalaureate degree registered nursing program.
• Bachelor of Science in Nursing
• Current Registered Nurse License in the State of Texas or Multi‑State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date)
• Case Management certification desirable
• Previous work experience in a related field with two years as a minimum
