Job summary

HCA Houston Healthcare Medical Center in Houston, TX seeks an RN Case Manager PRN to support patients and families across the care continuum. You will coordinate services, develop care plans, and partner with physicians, nurses, and leaders to ensure safe transitions and quality care.

Required: Bachelor’s in Nursing (Master’s desirable), 5 years acute care, 2+ years utilization review/case management, 1 year discharge planning; TX RN License. Benefits offered; PRN opportunities available.

Qualifications

• Bachelor’s degree in Nursing preferred; Master’s Degree desirable or commensurate experience
• Minimum of five (5) years recent acute care experience required
• Minimum of two (2) years of utilization review/case management experience and minimum of one (1) year experience in discharge planning in an acute care setting highly desirable
• TX RN License required

Responsibilities

• Guiding patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans reflecting patient strengths, needs, and recovery goals
• Completing psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team
• Collaborating with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless, patient-centered care
• Promoting HCA Healthcare’s values through every patient and family interaction

Job Summary and Qualifications

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Your responsibilities will include:

• Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights

• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
