• Design and adjust treatment plans based on patient progress and goals.
• Prepare for discharge with organized documentation and equipment needs.
• Collaborate with the care team to ensure effective patient treatment and communication.
• Maintain accurate records of progress and treatment plans.
• Perform other duties as assigned.
