Job summary

Sweetwater Care is seeking an experienced LVN in California to lead nursing operations within a long-term care facility. The role includes implementing infection control, coordinating quality improvement, and monitoring patient placement and care plans.

You will educate families and patients, administer medications and treatments per policy, and collaborate with physicians during rounds. California LVN licensure and 1+ year management exposure are preferred.

Qualifications

• California LVN license required.
• Prior nursing management experience preferred.
• SNF/LTC experience preferred.
• Must provide LVN license number.

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Utilizes Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.
