Job summary

University Health Careers is seeking a Service Coordinator-Licensed Vocational Nurse (SC-LVN) in Texas to collaborate with medical, behavioral, and social support staff to holistically evaluate member needs and preferences. You will educate members and their networks while coordinating LTSS services within a managed care framework.

The role requires at least three years of clinical nursing experience, with preferred experience in managed care, data entry, and RUG/MDS/PASRR.

Qualifications

• Three years of clinical nursing experience minimum.
• Experience in a managed care environment or with people who are disabled or vulnerable with chronic/complex conditions preferred.
• Data entry experience is required.
• Call center experience preferred.
• Experience with RUG/MDS/PASRR preferred.

Responsibilities

• Collaborate with medical, behavioral, and social support staff to evaluate STAR+PLUS members’ needs and preferences.
• Provide education and health information to the member, the member’s representative, and the member’s support network.
• Coordinate LTSS services for STAR+PLUS members, addressing physical, behavioral, functional, and psychosocial needs.
• Ensure compliance with Community First Health Plans policies and protocols and maintain good working relationships with coworkers, members, and guests.
