University Health is seeking a Service Coordinator-Licensed Vocational Nurse to partner with medical, behavioral, and social support staff. You will assess STAR Kids Members’ needs and coordinate services across acute care and LTSS with a holistic approach.

The role emphasizes education for the Member, the LAR, and the Support Network, while ensuring compliance with internal policies and fostering collaboration within the Health System.

Formación

• Minimum of three years of clinical nursing experience.
• Two+ years of experience in a managed care environment or with people with disabilities and vulnerable populations with chronic/complex conditions.
• Data entry experience required.
• Call center experience preferred.

Responsabilidades

• Collaborate with medical, behavioral, and social support staff to evaluate members’ needs and preferences.
• Coordinate STAR Kids members’ covered and uncovered services, including LTSS and acute care.
• Educate the member, the LAR, and the support network and maintain policy-compliant workflows.
