Job summary

University Health Careers in Texas seeks a Service Coordinator-Licensed Vocational Nurse (SC-LVN) to support STAR+PLUS Members by coordinating medical, behavioral, and social services, assessing needs, and educating the Member, their Representative, and Support Network.

The SC-LVN collaborates across teams to ensure timely access to LTSS, meets physical, behavioral, functional, and psychosocial needs, and adheres to Community First Health Plans policies while maintaining strong relationships

Qualifications

• Minimum three years of clinical nursing experience preferred.
• At least two years of experience in a managed care environment or working with people with disabilities and vulnerable populations with chronic or complex conditions preferred.
• Data entry experience is required.
• Call center experience preferred.
• Experience with RUG/MDS/PASRR preferred.

Responsibilities

• Collaborates with medical, behavioral, and social support staff to evaluate Members’ needs and preferences and provide health education to the Member, Representative, and Support Network.
• Coordinates STAR+PLUS LTSS services for Members, addressing physical, behavioral, functional, and psychosocial needs.
• Ensures compliance with policies and procedures and maintains good working relationships with Members, co-workers, and guests.

POSITION SUMMARY/RESPONSIBILITIES

The Service Coordinator-Licensed Vocational Nurse (SC-LVN) collaborates with medical, behavioral, and social support staff within the Service Coordination team to provide a holistic evaluation of STAR+PLUS Members’ individual dynamics, needs, and preferences while providing education and health-related information to the Member, the Member’s Representative, and the Member’s Support Network. Coordinates STAR +PLUS Members covered and uncovered Long Term Services and Supports (LTSS) services, while meeting the Member's physical, behavioral, functional, and psychosocial needs. Complies with Community First Health Plans, Inc. (Community First) policies, procedures, and protocols for establishing and maintaining good working relationships with co-workers, Members, and guests

EDUCATION/EXPERIENCE

A minimum of three years of clinical nursing experience is preferred. At least two years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions is preferred. Data entry experience is required. Call center experience preferred. Experience with RUG/MDS/PASRR preferred

LICENSURE

Current unrestricted license in the State of Texas as an LVN is required
