Job summary

University Health is seeking a Service Coordinator-Licensed Vocational Nurse (SC-LVN) to collaborate with medical, behavioral, and social support staff. You will evaluate members and coordinate LTSS services to meet their comprehensive needs.

Qualifications

Three years of clinical nursing experience preferred.
Two years in a managed care environment or with people with disabilities needing chronic/complex care preferred.
Data entry experience required.
Call center experience preferred.
Experience with RUG/MDS/PASRR preferred.

Responsibilities

Collaborate with medical, behavioral, and social support staff to evaluate members’ needs and preferences.
Provide education and health-related information to the member, the member’s representative, and the member’s support network.
Coordinate STAR+PLUS LTSS services to meet physical, behavioral, functional, and psychosocial needs.
Comply with Community First Health Plans policies, procedures, and protocols while maintaining good working relationships with coworkers, members, and guests.
