Job summary

Oakwood Gardens Care Center in California is seeking a Licensed Vocational Nurse (LVN) for the Full-Time PM shift. The role provides direct nursing care, supervises CNAs, administers medications, and updates resident records.

Ideal candidates are graduates of an approved LVN/LPN program with an active California LVN license and CPR certification. Long-term care experience is preferred; PCC knowledge is a plus and strong communication and teamwork are essential.

Qualifications

Responsibilities

• Provide direct nursing care in accordance with resident assessments and care plans.
• Supervise and support CNAs to ensure quality care delivery.
• Administer medications and treatments as ordered by physicians.
• Accurately document nursing care, medication administration, and resident status.
• Monitor residents for changes in condition and report findings promptly.
• Participate in admissions, transfers, and discharges as assigned.
• Ensure compliance with infection control, safety, and sanitation standards.
• Communicate effectively with residents, families, physicians, and interdisciplinary team members.
• Assist with restorative and rehabilitative nursing programs.
• Respond appropriately to emergencies and resident needs.
• Maintain resident confidentiality and comply with HIPAA regulations.

Job Title: Licensed Vocational Nurse (LVN) – Full-Time PM Shift Facility: Oakwood Gardens Care Center Shift: PM Shift Salary Range: $28.00 – $30.00 per hour (BOE)

Essential Duties & Responsibilities

• Provide direct nursing care in accordance with resident assessments and care plans
• Supervise and support CNAs to ensure quality care delivery
• Administer medications and treatments as ordered by physicians
• Accurately document nursing care, medication administration, and resident status

Qualifications

• Current, active, and unencumbered LVN license in the State of California
• Current CPR certification
• Knowledge of PCC preferred
