Job summary

Oakwood Gardens Care Center is seeking a Licensed Vocational Nurse (LVN) for the Full-Time PM Shift. You will provide direct nursing care to residents, supervise CNAs, and administer medications as ordered by physicians.

The ideal candidate graduates from an LVN/LPN program with long-term care experience. California LVN license and CPR certification are preferred; PCC knowledge is a plus. The facility emphasizes HIPAA compliance and resident-centered care.

Qualifications

Responsibilities

  • Provide direct nursing care in accordance with resident assessments and care plans.
  • Supervise and support CNAs to ensure quality care delivery.
  • Administer medications and treatments as ordered by physicians.
  • Document nursing care, medication administration, and resident status.
  • Monitor residents for changes in condition and report findings promptly.
  • Participate in admissions, transfers, and discharges as assigned.
  • Ensure compliance with infection control, safety, and sanitation standards.
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members.
  • Assist with restorative and rehabilitative nursing programs.
  • Respond to emergencies and resident needs.

Job Title: Licensed Vocational Nurse (LVN) – Full-Time PM Shift Facility: Oakwood Gardens Care Center Shift: PM Shift Salary Range: $28.00 – $30.00 per hour (BOE)

Essential Duties & Responsibilities
  • Provide direct nursing care in accordance with resident assessments and care plans
  • Supervise and support CNAs to ensure quality care delivery
  • Administer medications and treatments as ordered by physicians
  • Accurately document nursing care, medication administration, and resident status
  • Monitor residents for changes in condition and report findings promptly
  • Participate in admissions, transfers, and discharges as assigned
  • Ensure compliance with infection control, safety, and sanitation standards
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members
  • Assist with restorative and rehabilitative nursing programs
  • Respond appropriately to emergencies and resident needs
  • Maintain resident confidentiality and comply with HIPAA regulations
  • Provides direct supervision to CNAs during assigned shift
  • Ensures staff adhere to policies, procedures, and scope of practice
Qualifications Education & Experience
  • Graduate of an approved LVN/LPN program
  • Long-term care or skilled nursing experience preferred
Licenses & Certifications
  • Current, active, and unencumbered LVN license in the State of California
  • Current CPR certification
  • Knowledge of PCC preferred

Physical Demands

  • Exposure to residents, visitors, and healthcare equipment
  • Noise level generally low to moderate
