CenterWell Home Health, part of Humana, is seeking a Patient Services Coordinator-LVN in Fort Worth, TX. You will manage patient schedules, coordinate with field staff and physicians, and support daily office operations to ensure continuity of care.

The role requires an active LVN/LPN license in Texas, at least 1 year in home health, strong communication, and organizational skills. Clinical duties may include patient visits and on-call rotation.

Qualifikationen

• Must be a Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state of practice.
• At least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.
• Must possess a valid state driver's license and automobile liability insurance.
• Must be currently licensed in the State of employment if applicable.
• Must possess excellent communication skills, the ability to interact well with a diverse group, strong organizational skills, and the ability to manage and prioritize multiple assignments.

Aufgaben

• Schedules visits and manages patient schedules.
• Initiates infection control forms and routes to HRD for upload.
• Acts as back-up during lunch hours and busy periods; handles field calls and case conferences.
• Maintains the client hospitalization log and shares coordination notes with staff.
• Ensures staff are scheduled for skilled nurse/injection visits and add-ons; reschedules as needed.
• Runs scheduling reports (Agent Summary, Missed Visits); prepares weekly agent schedules for director approval.
• Verifies visit paper notes and assists with internal transfer of patients between branches.
• If clinical, reviews lab reports for normality and routes to appropriate departments; may perform patient visits or participate in on-call rotation.
