Job summary

First Place Pediatrics is seeking a dedicated home health nurse to provide 1:1 patient care in home settings for pediatric and adult patients. You will work with patients requiring g-tube, trach, and ventilator support, documenting your progress electronically and performing regular assessments.

Qualifications

• Graduate of an approved nursing program and licensed in the applicable state.
• Current CPR certification.
• Pediatric experience or g-tube/trach/vent experience preferred.

Responsibilities

• Provide 1:1 patient care in home settings.
• Care for pediatric and adult patients including g-tube, trach, and ventilator support.
• Document care using the EMR platform.
• Perform patient assessments and adjust care plans accordingly.
• Build meaningful, long-term relationships with patients and families.

Throughout the day you will utilize your skills independently to provide 1:1 care to your patient.

Qualifications, Licenses, and Certifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• Maintains a current CPR certification
• BLS (Basic Life Support) Certification (preferred)
• Pediatric experience (preferred)
• G-tube, Trach, and Ventilator experience strongly preferred
