Job summary

First Place Pediatrics LLC is hiring for home health nursing to provide 1:1 in-home care for pediatric and adult patients in the Garden Grove area. You will assess needs, administer medications, and coordinate care using an EMR system, delivering compassionate, consistent service in the patient’s home.

Responsibilities include in-person visits, documenting progress, and building lasting relationships with patients and families while collaborating with the care team and providers.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) where practicing.
• Current CPR certification.
• BLS certification preferred.
• Pediatric experience preferred.

Responsibilities

• 1:1 patient care.
• Work with pediatric/adult patients with g-tube, trach, and/or ventilator.
• Document electronically using EMR.
• Conduct patient assessments.
• Provide in-person home health services.
• Develop meaningful long-term relationships with patients and families.
